Charlie\u2019s insight into life after an accident is inspiring to learn. Apply the safety lessons now; not after something happens to you. He completely accepts responsibility for the accident and does not play the role of a victim. \u201cYou had control, you made choices, and you caused the accident.\u201d<\/p>\n

I look forward to writing a case study-based on the book to use in my human factors classes. Many of the elements of courses I teach were present and detectable in advance in Charlie’s story.<\/p>\n

My favorite quote from Charlie is \u201cThat\u2019s the problem with safety: when we take personal responsibility out of the equation, what we put in its place are practices and procedures and solutions that are often bizarre in the lengths they go to apologize for basic human stupidity.\u201d<\/p>\n

I have seen that too many times in aviation. Let\u2019s write a new policy or procedure for the choice of an individual. Flight Operations Manuals are getting way to long trying to replace good decision making and risk management.<\/p>\n

In his book, Out of the Ashes<\/a>, Charlie Morecraft reflects back on an accident at the refinery where he worked that nearly cost him his life. Charlie had years of experience on the job and was the poster child for an accident waiting to happen. Sleeping through safety classes, not wearing his safety equipment, and taking any shortcut in procedures he could dream up made him lucky he did not kill himself or someone else years before.<\/p>\n

The accident opened his eyes to the broad implications of an accident. \u201cI absolutely believed that an accident would never happen to me \u2013 ever. And even if it did, I reasoned, the only one who would be hurt by it would be me alone. Not my kids, my parents, or my friends. Not my colleagues at the refinery or my bosses or the other guys who counted on me to lead by example.\u201d<\/p>\n
