At the closest point the two aircraft were 0.82 nautical miles apart laterally and 700 feet vertically. No damage, no injuries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Per the NTSB\u2019s preliminary report (accident number OPS26LA063, investigation still open), a radio line-of-sight problem from the helicopter\u2019s temporary site, a pre-departure notification call that arrived three minutes late, and a takeoff clearance issued without tower awareness of the helicopter all lined up at once. The Envoy crew got a TCAS advisory and never saw the helicopter visually; the helicopter crew saw the jet and held.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It\u2019s the same hazard pattern behind the January 2025 Reagan National collision that killed 67 and led the FAA to make its DCA helicopter route restrictions permanent, a reminder that the gap can still open even with those restrictions in place. This time, TCAS and see-and-avoid held the line the notification chain didn\u2019t.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We built a new case study on this event into our Mid-Air Collision Avoidance courses, both for fixed-wing and rotor-wing, each from its crew\u2019s perspective.
