The book does not go into the level of detail to learn the intricacies of each of his significant contributions to the field of human error, but places the journey in perspective. Then you can go back and deep dive into the theories that define how we look at human error today in aviation, including the widely known Swiss Cheese model. Chapter 14 highlights who actually came up with that winning label for Dr. Reason\u2019s model.<\/p>\n","protected":false},"excerpt":{"rendered":"<p><img decoding=\"async\" style=\"margin: 5px; float: right;\" alt=\"a life in error\" src=\"images\/a_life_in_error.png\" \/>Dr. James Reason\u2019s work is often cited when the topic of human error comes in up aviation. Dr. Reason has published a long list of books on human air, including his most recent \u201cA Life in Error: From Little Slips to big Disasters\u201d (my use of the incorrect \u201cair\u201d versus \u201cerror\u201d can be explained in Chapter 7: The Freudian Slip Revisited).<\/p>\n<p>I own most of Dr. Reason\u2019s books, although this most recent is the first I have finished reading. It is only after I read or hear someone else\u2019s explanation of what Dr. Reason said in one of his books that I understand it. I agree with Erik Hollnagel, who wrote the forward to the book, that \u201cJim is an undisputed master of the English language.\u201d I am at the opposite end of that spectrum and often get lost in his masterful use of language and put down his books sooner than I should. I was glad to read that Dr. Reason\u2019s perspective on the book \u201cHuman Error\u201d was that it was written for psychologists and was full of \u201cpsychobabble.\u201d I was lost at the first babbling brook.<\/p>\n<p>\u201cA Life in Error\u201d was a much easier read for me and placed his other books in context for me. Being a holistic type learner, knowing the big picture of his progression of research into human error will better equip me to go back and reread his past work.<\/p>\n<p>I was distracted while reading the book when I noticed at the end of Chapter 3 it cited the next Chapter as Chapter 5. I wondered if this was a carefully placed typo which would be used later in the book as an example. After noticing that the reference to Chapter 3 later in the book actually meant to cite Chapter 2 and references to Chapter 4 were actually 3, I began to ponder if these typos could be attributed to a slip, lapse, or error.
